Hospital to Home, helping bereaved parents navigate to ongoing support
For the second webinar in the series, please join us for an evening focusing on the incredible Hospital to Home program. Hear how the program was developed from a pilot to an innovative support intervention that is highly regarded by bereaved parents. Evidence shows promising effects on wellbeing for parents following the death of a baby.
A program operating throughout Australia, Hospital to Home provides outreach support within three months of loss to bereaved parents who have experienced the death of a baby or infant through stillbirth, neonatal death or the sudden unexpected death of an infant (SUDI).
